Family Medical Leave

Family Medical Leave (FML) is one of the most misunderstood benefits of employees and supervisors. The following is a checklist to inform you about the benefits of FML and the information that is required from the employee.

• Basic Family Medical Leave information
• FML Request form must be completed for absences of more than 3 days.
• A Physician's Certification must be completed for all FML events (except regencies)
• Family Medical Leave without pay form
