Procedures to be followed by the employee when making a Training Account request:
- Complete Section I of the Training and Development Request Form (T&D Request Form) at least 3 weeks prior to the training event being requested.
- Discuss your request with your supervisor and, if s/he approves it, the supervisor signs Section II of the form and indicates the account to be charged for your salary.
- If the supervisor denies the request, s/he checks the "denied" box, indicates the reason and returns the form to you.
- In either case (approval or denial), give the completed T&D Request Form, along with the other required forms (Travel Request Authorization, Purchase Requisition) to the Training Coordinator (TC) for review and signature.
- If approved, the TC signs the form and sends them to the Director's Office for final authorization. If the TC cannot approve the expenditure as requested, the TC will notify you and your supervisor in writing as soon as possible.
- After receiving the Director's signature, submit the completed forms to the Business Office for processing.
- You are responsible for coordinating with the Business Office your registration, materials acquisition, travel arrangements, and the completion of your expense report.
- Return the completed GI Evaluation Form to the TC as soon as you return from your training event.
