Registration

Workshop registration may be completed by filling out the Word (65 kb) or pdf (160 kb) Registration Form. Payment should be made in US dollars and may be received in cash, checks drawn from a US bank, or travelers' check, and may be made in advance or at the Workshop. Checks should be made out to Geophysical Institute. Even if you intend to pay at the Workshop, please submit a Registration Form in advance for planning purposes.

Both the PDF and Word versions are fully editable. In the Word version it will help if you toggle the overwrite feature prior to editing (hit the INSERT key).

Payment may also be made via credit card (VISA or MASTERCARD only). When paying by credit card, be sure to include your ZIP code (US residents only) and 3-digit CVV/CVC code printed on the signature portion of your card. A signature is not required if the registration form is sent via email/fax/post and the CVV/CVC code is provided.
