Employee Recognition Nomination Form Selection
At this time you must use the printed form for your nomination. Forms are available outside the Human Resources Office on the sixth floor of the Elvey building and at Attorney's Plaza. The first thing you must do to make a nomination is decide whether the nominee is a student employee or a staff employee. As this site is updated you will be able to make your nominations online.

After you have completed the appropriate form put it into Jesse Atencio's mailbox in the mail room. You will receive confirmation of receipt of your nomination via e-mail. All nomination information will be held confidential by the Employee Recognition Committee. If you want the person you nominate to know about the nomination, you will have to inform them yourself.
Deadlines for nominations are January 1 and July 1. Nominations not selected will be discarded and not carried over to the next deadline. Therefore, if you nominate someone that is not selected you will have to resubmit your nomination if you want them considered again.
IMPORTANT!! It is important that you take the time to supply as much information as you can on the nomination form. It is wonderful that you are taking the time to nominate someone but the committee can only consider the information you supply on the form in their selection process. If you are taking the time to nominate someone, please take more than 30 seconds to write it up.
